Your guide to registering for South Eastern Community Connect (SECC) services:

  1. Contact My Aged Care on 1800 200 422
  2. Request the referral code for the service(s) you need. Please see below for the list of service types.

Note: You will need a separate referral code for each type of service, so you may be requesting multiple codes.

We can help with the following service types:

  • Community Transport (individual transportation to medical appointments or other appointments or group transportation on our Shopping Bus Service. This is generally a pick up/ drop off service)
  • Social Support Group (group bus trips or attendance at The Cottage Dementia Day Centre)
  • Social Support Individual (Individual social support in your home or to accompany you in the community. Please note that this does not include personal care/ support with showering)
  • Flexible Respite (individual support provided to enable a primary carer a break from their caring responsibilities- this may occur in the home or in the community or a combination of both)
  • Other Food Services (home delivery of fresh fruit and vegetable boxes or individual support provided in your home to support you to prepare meals)
  • Home Care Packages (individualised packages of support).

Morning tea locations are subject to weather conditions.

With the Shopping Bus to Eastgardens on Fridays, you now have the option of attending for either 1.5 or 3 hours.
