Aged and Reablement Services

We’re here to help you with the personalised support you need.

Whether it’s help with shopping or making new friends, we’d love to talk with you about how we can help.

My Aged Care

Your pathway to government funding for these services is My Aged Care. My Aged Care (MAC) is accessible via the website or by calling 1800 200 422.

Through My Aged Care, the Australian Government helps you navigate the aged care system, gives you choice, more control, and easier access to a full range of aged care services.

Since 1 July 2015, new service users aged 65 years or older have needed to register with My Aged Care (MAC) to receive help from the government to pay for any of these services.

To make a referral to South Eastern Community Connect, please contact My Aged Care and nominate us as a preferred service provider.

Should you choose to pay for these services in full yourself, please contact us directly to book.

Carer Gateway

Carer Gateway makes it easier for carers to access practical information and advice and connect with services in their local area. Carer Gateway comprises a national telephone service, website and an interactive service finder to help carers locate their nearest existing support services. Carers can call 1800 422 737 from Monday to Friday between 8am and 6pm and speak to a helpful and professional Australia-based team who can help with information and provide links to local services.

You can help create a better future for your community.
